Red Tapes for New Parents and Newly Weds

Call it being lazy in my part for copying and pasting the following information received from our Human Resources Department only but I think they are so important that I wanted to keep a copy in my blog for future reference… wish ko langggg. :)

So anyway, here goes…

*************************

In the past few months, a lot of our colleagues became new parents.

New Fathers and New Mothers. Congratulations!!!

There were also some who changed their Civil Statuses.

However, not all of these new parents and/or newlyweds were able to avail the benefits due them by the government.

Example:
Based on the SSS requirements; Maternity/Paternity notification should be submitted, and received, by SSS before any claims/benefits can be availed.

Call it misinformed, or just plain ignorance, but some were denied Maternity/Paternity claims simply because they failed to submit any notification to SSS.

Even with much pleadings and justifications, SSS will not entertain any claims/benefits if no notification was received by them.

Therefore, we would like to remind everybody of these responsibilities;

SSS
Women = the moment you know you are pregnant, immediately file a Maternity notification.

Men = the moment you know your wife/partner is pregnant, immediately file a Paternity notification.

New Parents = file an SSS-E4 to amend/update your member’s data to include your new child as dependent.

Newly Married = file an SSS-E4 to amend/update your member’s data to reflect Change of Civil Status.

Philhealth
New Parents = file a Philhealth PMRF to amend/update your member’s data record (MDR) to include your new child as dependent.

Newly Married = file a Philhealth PMRF to amend/update your member’s data record (MDR) to reflect Change of Civil Status.

BIR
New Parents = file a BIR 2305 to amend/update your tax exemption bracket.

Newly Married = file a BIR 2305 to amend/update your tax exemption bracket.

INSURANCE
New Parents = file an AON enrolment form to amend/update your dependents/beneficiaries.

Newly Married = file an AON enrolment form to amend/update your dependents/beneficiaries.

DSWD = Department of Social Welfare and Development
Solo Parent = apply for your Solo Parent ID at the DSWD office.
